Privacy Policy for USA Life Insure
Effective Date: 9/1/2026
At USA Life Insure, we believe your privacy and information security are important. This Privacy Policy explains how we collect, use, share, and protect information when you use www.usalifeinsure.com, and describes the choices and rights available to you. If you have questions, contact us at 512-236-5088 or the address at the end of this policy.
1. Information We Collect
1.1 Insurance Quote Request Form
To provide accurate life insurance quotes, we collect the following: state of residence, date of birth, gender, height, weight, desired coverage amount, desired term period or length, health classification, and tobacco use. Simplified-issue and guaranteed-issue quotes may require additional health information. This data is required because life insurance carriers set rates based on these factors.
1.2 Insurance Application Request Form
This form initiates the insurance purchase process and requests contact from a member of our staff. We collect your name, mailing address, email address, and phone number(s), used only to contact you about your insurance purchase. We do not sell this information; we may share it with life insurance carriers and the service providers described in Section 4 to help you obtain coverage. Additional health and lifestyle information required by insurance applications does not need to be submitted online and is collected later in the application process.
1.3 Automatically Collected Information
Our servers automatically log the IP address of the device contacting us, the date and time of the visit, the referring site, and the browser, operating system, and screen resolution information. We use this data in aggregate to understand site usage, identify popular pages, and improve the Site. We do not use log files to identify individual visitors or to collect personal information.
1.4 Cookies and Similar Technologies
A cookie is a small piece of data stored on your device. We use session cookies (deleted when you close your browser) and persistent cookies (stored for a set period) to measure site effectiveness. None of our cookies are linked to personally identifiable information.
2. How We Use Your Information
- To generate real-time, accurate life insurance quotes.
- To initiate and process life insurance (and related product) applications.
- To contact you about your quote or application by phone, email, mail, fax, or text, consistent with the consent you provide (see Section 3).
- To analyze aggregate site usage and improve the Site.
- To comply with legal, regulatory, and insurance-industry recordkeeping obligations.
3. Telephone, Text, and Email Consent (TCPA)
By submitting your phone number on our quote or application forms, you expressly consent to be contacted by USA Life Insure, and by the insurance carriers and partners identified in Section 4, at the number provided — including by telephone call, SMS/text message, automatic telephone dialing system, or prerecorded or artificial voice message — regarding life insurance products and services, even if your number is on a state or federal Do-Not-Call registry.
- Consent is not a condition of purchasing any insurance product or service.
- Message and data rates may apply to SMS/text communications.
- You may revoke consent at any time by replying “STOP” to any text, saying “remove me” on a call, emailing us, or calling 512-236-5088. We will honor opt-out requests promptly and add you to our internal Do Not Contact list, which is maintained for at least 10 years, as required by the Telemarketing Sales Rule.
- We do not knowingly call or text reassigned numbers; if you receive a call or text intended for someone else, please let us know so we can update our records.
This consent language should appear as a clear, conspicuous disclosure directly above or below the phone number field and any “Submit” or “Get My Quote” button — not only inside this policy — to meet TCPA’s proximity and clarity requirements.
4. How We Share Information
We do not sell your personal information for money. We may share the information described in Section 1 with:
- Life insurance carriers, to obtain quotes and process applications on your behalf.
- Service providers who help us operate the Site, process applications, or communicate with you, under contractual confidentiality obligations.
- Regulators or other parties where required by law.
USA Life Insure is not responsible for the privacy practices of third-party carriers, partners, vendors, or affiliates. Please review their privacy policies directly.
5. Data Retention and Security
We retain quote and application data for as long as needed to service your insurance request, comply with insurance recordkeeping and TCPA/TSR requirements, and resolve disputes, after which it is securely deleted or de-identified. We use reasonable administrative, technical, and physical safeguards to protect your information, though no method of transmission or storage is completely secure.
6. California Privacy Rights (CCPA/CPRA)
6.1 Categories of Personal Information Collected
In the past 12 months, we have collected the following categories of personal information, as defined by the CCPA: identifiers (name, address, email, phone); characteristics protected under California or federal law (age, sex); commercial information (insurance products inquired about); internet/network activity (log data, cookies); and sensitive personal information (health information such as height, weight, tobacco use, and health classification).
6.2 Your Rights
- Right to Know — request the categories and specific pieces of personal information we’ve collected, used, and disclosed.
- Right to Delete — request deletion of your personal information, subject to legal exceptions (e.g., active applications, regulatory recordkeeping).
- Right to Correct — request correction of inaccurate personal information.
- Right to Opt Out of Sale/Sharing — opt out of any sale or sharing of your personal information, including for cross-context behavioral advertising.
- Right to Limit Use of Sensitive Personal Information — limit our use of health-related information to what’s necessary to provide the insurance quote or application you requested.

6.3 How to Exercise Your Rights
Submit a request by calling our direct number, 512-236-5088, or by emailing sbenson@usalifeinsure.com. We will verify your identity before processing your request and will respond within 45 days, as required by law. You may also designate an authorized agent to submit a request on your behalf.
7. European Privacy Rights (GDPR)
Where the General Data Protection Regulation (GDPR) applies to your use of the Site, we process your personal information on the following legal bases: your consent (for cookies and marketing contact), the necessity of processing to take steps toward a contract at your request (for quote and application processing), and our legitimate interests in operating and improving the Site. Health-related information is a “special category” of data under GDPR and is processed only with your explicit consent.
You have the right to: access the personal information we hold about you; request correction or erasure; restrict or object to processing; receive your data in a portable format; and lodge a complaint with your local data protection supervisory authority. To exercise these rights, contact us at sbenson@usalifeinsure.com.
8. Cookie Consent
Where opt-in consent is legally required (including for EU/UK visitors), non-essential cookies are not set until you affirmatively consent via our cookie banner. You may withdraw consent or change your preferences at any time through the cookie settings link in the Site footer, or by adjusting your browser settings. Rejecting cookies will not prevent you from using the Site.
9. Communications With Clients
Once you initiate a relationship with USA Life Insure the Site, we may contact you by telephone, email, mail, or fax to help you apply for, purchase, or maintain life insurance or related products (such as annuities, health, or vision coverage), consistent with the consent described in Section 3. We are not responsible for the privacy practices or content of those third-party sites. We encourage you to review the privacy policy of any third-party site before providing your information.
11. Children’s Privacy
The Site is not directed to, and we do not knowingly collect personal information from, individuals under the age of 16. If we learn that we have inadvertently collected personal information from a child under 16 without verifiable parental consent, we will promptly delete that information. If you believe we may have collected information from a child under 16, please contact us using the information in Section 13.
